Medical records
- If you were last seen between 2013 and 2019, CDH alone has these records. Email records requests to cdh@dizzy-doc.com.
- Medical records involving visits to CDH between 2019 through 4/2023, are stored in Northwestern Medicine's Epic electronic medical record system. CDH was not provided with a full copy of the Northwestern records. To obtain Northwestern records, you will need to contact their records department.
